Dudley Group NHS Foundation Trust is seeking an experienced multidisciplinary team assessor to support NHS Continuing Healthcare and Funding Nursing Care eligibility assessments across community, home, and hospital settings.

You will use national assessment tools, coordinate with the MDT, explain the process to patients and families, and ensure reviews are completed in line with current guidance.

Qualifications

• Assess individuals in community/home/hospitals for NHS Continuing Healthcare eligibility.
• Coordinate assessment process with the multidisciplinary team (MDT).
• Explain the process clearly to patients, families and care providers.

Responsabilités

• Utilise national assessment tools and adhere to required timescales.
• Ensure reviews of NHS Funded Care are conducted in line with current guidance.
