Job summary

HCRG Care Group is seeking a dedicated Care Navigator for our Neurodevelopmental Pathway across B&NES or Wiltshire, combining hub work, regional visits and some remote duties. You will guide families through referrals for ADHD and sleep supports, collecting information to aid clinical decisions.

You’ll work with schools, health and voluntary partners to ensure timely access to services, delivering needs-led support and signposting as needed.

Qualifications

• Educated to at least level 4 qualification in relevant area.
• Strong interpersonal and communication abilities with empathy.
• Experience working with autism and other neurodevelopmental conditions.

Responsibilities

• Assist families and professionals to understand referrals for ADHD/autism support.
• Gather information from families and young people to support clinical decisions.
• Ensure access to recommended services and provide enhanced support to those not yet engaged.
• Collaborate with partner agencies and parent carer organisations to coordinate services.
• Provide needs-led calls and clear signposting to appropriate support.
