Qualifications

• 5 years post‑registration experience in neonatal or transitional care.
• Active NMC registration and UK practice readiness.
• Experience with safeguarding policies and procedures.

Responsibilities

• Lead clinical oversight of the transitional care service.
• Develop, implement and monitor individualized care plans for transitional care.
• Collaborate with multidisciplinary teams to support early discharge.
• Champion family-integrated care and parental involvement.
• Act as clinical educator and mentor for nursing and midwifery staff.
• Drive quality improvement and audits to raise care standards.
• Maintain infection control, documentation and safeguarding practices.
• Coordinate workforce planning, rota development and staff development.
• Contribute to scoping and potential Neonatal Outreach Team implementation.
