Job summary

Leeds Community Healthcare NHS Trust is recruiting a Neighbourhood Clinical Assistant to join our Neighbourhood Teams. The role covers Wetherby, Meanwood and Chapeltown areas with 8am–8pm shifts and flexible options to fit busy lives.

You will work in patients’ homes within a multidisciplinary team, providing clinical support and tailoring induction and ongoing development with team members from various professions. The role is based in Leeds and involves community-based care.

Responsibilities

• Assess and respond to changing patient needs within your own clinical limits and seek support from the wider clinical team when appropriate.
• Provide patients with clinical support, for example administering medications, undertaking wound care and carrying out observations.
• Work in a multidisciplinary team delivering care to patients in their own home, contributing to a varied caseload with flexible shift patterns.
