Job summary

Provide Community is seeking a community healthcare post holder to work with the community matron in planning, allocating and reviewing patient care across Maldon ICT bases. The role involves caseload management, care plan formulation and evaluation, and supporting other bases as needed.

Responsibilities

• Caseload management, including allocation and review of visit frequency and prioritising care needs.
• Formulation and review of individual care plans.
• Evaluation of care including initial home visits and ongoing assessment.
• Ability to problem solve and adapt plans to changing patient needs.
• Promote health by educating patients and carers in line with the service philosophy.
• Liaise with the primary care team, social services and other agencies to coordinate care.
• Participate in care coordination and complex discharges home, including palliative care.
• Maintain/develop knowledge and competence per NMC Code of Professional Conduct and PREPP.
• Stay informed on current primary care issues and political changes.
• Develop and promote effective multi-disciplinary communication networks.
