Job summary

The post holder at Provide CIC, NHS site Eastlight House, Braintree, will work with the Community Matron to plan, allocate and review patient care for a defined caseload. This role is based in Braintree and may require travel within the team’s bases.

Key duties include caseload management, care plan formulation, evaluation of care, promotion of health, and liaison with the primary care team and social services to coordinate multi-disciplinary care.

Qualifications

• Current NMC registration and ongoing professional development.
• Relevant nursing degree or equivalent community qualification.
• Qualified Nurse Prescriber or working towards.
• Specialist community practitioner or equivalent qualification.

Responsibilities

• Caseload management including allocation and review of visit frequency.
• Formulation and review of care plans.
• Evaluation of care including first visits.
• Promote health through patient and carer education.
• Liaise with primary care, social services and other agencies.
• Develop care co-ordination and complex discharges including palliative care.
• Maintain knowledge and competence to meet patients' needs per NMC PREPP.
• Develop multi-disciplinary communication networks.
