Job summary

Your Place is seeking a Move-On Team Leader to provide day-to-day leadership across the Move-On Team, coordinating move-on and tenancy sustainment for residents. You will develop pathways that start early, support independent living, and oversee landlord partnerships and resettlement routes.

The role requires collaboration with residents, key-workers, managers and external partners to reduce barriers to move-on.

Qualifications

• Experience working with people affected by homelessness and move-on pathways.
• Experience supervising staff and coordinating workloads.
• Experience building landlord partnerships and housing pathways.

Responsibilities

• Lead day-to-day coordination of the Move-On Team and provide personalised move-on support.
• Develop and maintain a clear move-on pathway from assessment to post-move sustainment.
• Embed move-on planning into support planning and case reviews, providing specialist advice.
