Job summary

LOROS Hospice in Leicester seeks a Nurse Specialist to undertake home visits, lead MND clinics and provide holistic assessments. You will work with the multidisciplinary team to plan care and support patients and carers, maintaining records and complying with policy.

The role involves collaboration with the Hospice and external organisations, adherence to professional standards, and contributing to service development within the MND care pathway.

Responsibilities

• Provide nurse-led MND clinics.
• Offer telephone advice to patients, carers and professionals.
• Collaborate with multi-professional team to plan holistic care.
• Refer to other agencies when additional support is needed.
• Monitor progress of MND patients with MDT colleagues.
• Maintain electronic patient records according to policy.
• Comply with Health and Safety, Data Protection and organisational policies.
