Job summary

Pennine Care NHS Foundation Trust is recruiting a Mental Health Support Worker for the Memory Assessment Service in Stockport. The role involves providing direct care to service users in crisis, supporting diagnosis clinics, and developing post-diagnosis care pathways with the memory clinic team.

You will work with MDT colleagues to ensure timely care and appropriate referrals. The post emphasises collaboration, documentation, training, and patient/carer education, in a busy community mental

Qualifications

• Level 2 qualification or equivalent required, with further training/experience to Level 3
• Dementia training is desirable
• CST training is desirable

Responsibilities

• Support patients and carers in clinic following diagnosis and assist in care planning and signposting
• Work within post duties, seek help when unsure, report concerns to a supervisor
• Offer appropriate advice to carers and patients to reduce distress and promote wellbeing
• Contribute to memory assessment post-diagnosis pathway and support group sessions
• Attend team meetings and supervision and work with the MDT to ensure consistent care delivery
• Adhere to Trust policies and maintain accurate records and documentation
