• Support patients and carers in clinic following diagnosis and assist care planning.
• Work within limitations of the role, seek help when unsure and report concerns.
• Provide advice to carers and patients to reduce distress and promote wellness.
• Contribute to memory assessment pathways and post-diagnosis sessions.
• Assist with post-diagnosis groups and discharge planning.
• Document clear case notes and participate in supervision.
