Job summary

Community Care Live is seeking a qualified and experienced Social Worker to join our Mental Health Team. You will be innovative, person-centred and commit to strengths-based practice while supporting colleagues to continuous improvement.

The role involves complex caseloads, supervision of Case Workers, duty triage, safeguarding decisions and close liaison with Health for hospital discharges, with occasional AMHP work and progression potential.

Qualifications

• Qualified Social Worker with SWE registration.
• Strong strengths-based approach and practical application.
• Understanding of social work issues, legal frameworks and statutory duties.
• Commitment to demonstrating values and behaviours.

Responsibilities

• Conduct comprehensive assessments of individuals' needs and available support.
• Manage a complex caseload and provide supervision on complex cases.
• Triage urgent cases, safeguarding referrals and inform decisions.
• Liaise with Health colleagues to facilitate hospital discharges.
• Hold forensic cases and work with the Ministry of Justice.
