Job summary

Hertfordshire Partnership University NHS Foundation Trust is recruiting a Social Worker for the Adult Community Mental Health Services Team in North Hertfordshire. The role focuses on assessing, care planning and coordinating support for adults with mental ill-health within a community setting.

Qualifications

• Professional social worker registered with Social Work England
• Experience in adult community social work practice
• Knowledge of Care Act and safeguarding principles

Responsibilities

• Assess adults with mental ill-health and high-risk or complex cases against eligibility criteria
• Hold a caseload and deliver assessment, care planning and coordination of care
• Lead adult safeguarding practice and undertake safeguarding enquiries
• Carry out regular risk assessments to keep service users safe
• Undertake carer assessments and contingency planning
• Provide direct support and advice to service users and carers, signposting as needed
• Ensure commissioned services offer choice and apply charging processes
• Act as authorised signatory for care packages and manage service user funds
• Work in multi-disciplinary teams to develop recovery-focused community services
• Provide supervision to newly qualified social workers and unregistered staff (ASYE)
• Undertake AMHP duties or AMHP training and contribute to AMHP rota
• Chair case discussions and multidisciplinary meetings and maintain records
