Job summary

Lancashire and South Cumbria NHS Foundation Trust is seeking a Registered Mental Health Nurse, Social Worker or Occupational Therapist to join the Pennine West Home Based Treatment Team in Blackburn. The role provides care and treatment for service users in acute mental health crises at home with a compassionate, person-centred approach.

The post supports professional development, requires experience with complex mental health needs, and offers flexible working patterns within an integrated MDT

Qualifications

• Experience with collaborative care planning and engaging carers.
• Proven ability to undertake risk assessment and document risks.
• Experience working in a multidisciplinary team (MDT).
• Strong communication with service users, carers and teams.
• Willingness to work a 24-hour shift pattern.

Responsibilities

• Undertake collaborative care planning with service users and their carers.
• Undertake risk assessment and record risks in line with the trust policy
• Undertake shift co-ordination when allocated to do so.
• Participate in MDT and agree plans with patients in relation to their care.
• Maintain knowledge of evidence-based home treatment interventions and deliver these interventions.
• Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
• Undertake allocation of shift co-ordination.
• Demonstrate care that fosters patient engagement and supports service user and carer experience.
