The Methodist Church in Britain is seeking a Mental Health Practitioner to join the Pennine West Home Based Treatment Team. The role treats individuals in acute mental health crisis at home, collaborating with carers and a multidisciplinary team to plan care, assess risk and coordinate shifts.

Registration with NMC, HCPC or Social Work England is required, with experience in complex mental health needs and home treatment interventions. Flexible, travel-enabled and 24-hour shift work are involved.

Formación

• Registration as a mental health nurse with the NMC
• HCPC registration as an occupational therapist
• Social Work England registration as a social worker

Responsabilidades

• Plan care collaboratively with service users and carers
• Undertake risk assessments and record risks per policy
• Gatekeeping to ensure least restrictive care settings
• Deliver evidence-based home treatment interventions
• Provide psychosocial support during home treatment
• Coordinate shifts and allocate shift coordination
• Support carers during crisis
• Travel as required and work 24-hour shift patterns
