Job summary

Pennine Care NHS Foundation Trust is recruiting mental health practitioners to join the crisis resolution home treatment team in Tameside and Glossop, delivering specialist care in patients' homes as an alternative to hospital admission.

You will work within the MDT with psychiatrists and psychologists, carrying out holistic assessments, risk assessments, care planning and evidence-based interventions to promote recovery and safe discharge, with development opportunities in a values-driven trust

Responsibilities

• Provide holistic assessment, interventions, risk assessments and care plans for service users referred to the CRHTT.
• Assess health care needs and manage risk.
• Monitor and evaluate care plans.
• Liaise with the wider MDT.
• Provide evidence-based therapeutic interventions and psycho-education.
