London Borough of Camden is seeking a Mental Health Pathway Referrals Co-ordinator to help vulnerable residents access housing and support. You will work within the Pathways Move On Team to coordinate referrals into specialist mental health supported housing, partnering with housing, health and support services to move people into suitable accommodation swiftly.

This permanent role requires a proactive, empathetic approach, strong relationship-building, and ability to manage sensitive situations

Qualifications

• Strong ability to build relationships across housing, health and support services.
• Proven communication skills with people from diverse backgrounds.
• Excellent organisational skills and commitment to continuous improvement.
• Good experience using systems and data to support effective decision-making.

Responsabilités

• Coordinate referrals into specialist mental health supported housing with housing, health and support partners.
• Ensure move into appropriate accommodation as quickly as possible.
• Develop move-on opportunities and help prevent homelessness.
