Job description

This is an exciting opportunity to offer a Mental Health Navigator / Link Worker post within Community Mental Health (CMHT).

Key Responsibilities

• Work within a team of dedicated, friendly professionals committed to addressing wider determinants of health, such as debt, housing, and physical inactivity.
• Support individuals with serious mental illness (SMI) to connect with their communities and overcome barriers like social isolation.
• Engage effectively with diverse VCSE organisations, NHS teams, including primary care.
• Co-produce goals with individuals based on their priorities, interests, values, and motivations.

Location and Travel

Based at Weavers Croft alongside the clinical lead, primarily working in Gloucestershire with some out-of-county visits.

Role Details

• Collaborate with VCSE organisations and primary care to support individuals with or at risk of SMI.
• Participate actively in Locality Community Partnership meetings to plan and deliver collaborative care.
• Build capacity within VCSE organisations, including attending meetings, forums, and activities with clients.
• Enhance VCSE awareness and access to mental health services and training.
• Support early crisis identification and improve connectivity between mental health and third sector services.
• Support the personalisation agenda and connect GP practices with GHC mental health services.
• Manage a limited caseload to support referrals and broker access to services.
