Qualifikationen

• Undertake comprehensive mental health assessments for patients in ED and wards.
• Assess and manage risk, including self-harm and safeguarding.
• Provide expert mental health advice to acute hospital staff.
• Develop and review person-centred care plans and referrals.

Aufgaben

• Undertake comprehensive mental health assessments for patients presenting in the Emergency Department and acute/community wards.
• Assess and manage risk, including self-harm, suicide, vulnerability, and safeguarding concerns.
• Provide specialist mental health advice, consultation, and support to acute hospital staff.
• Develop, implement, and review person-centred plans and facilitate onward referrals.
• Work collaboratively with multidisciplinary teams to ensure coordinated and effective patient care.
• Contribute to the delivery of a responsive Mental Health Liaison Service operating 7 days a week.
• Maintain accurate clinical records and ensure compliance with relevant Health board pathways and legislation, including the Mental Health Act and Mental Capacity Act.
• Provide education, support, and training to healthcare professionals and students.
• Contribute to service development, quality improvement, and the promotion of best practice within the Mental Health Liaison Team.
