Job summary

Cwm Taf Morgannwg University Health Board is seeking a Band 6 Mental Health Liaison Practitioner to join the Mental Health Liaison Team at Prince Charles Hospital in Wales. The post involves coordinating comprehensive mental health assessments, risk management, and providing expert guidance to acute hospital staff to ensure timely, person-centred care.

The successful candidate will work across the Emergency Department and hospital wards within a 7-day service, including weekends and bank

Qualifications

• Liaison work experience or capability.
• Clinical assessment and formulation skills.
• Knowledge of safeguarding procedures.
• Welsh language speaking/listening at Level 3 or above.

Responsibilities

• Undertake comprehensive mental health assessments for patients in ED and hospital wards.
• Assess and manage risk, including self-harm, suicide, vulnerability, and safeguarding concerns.
• Provide specialist mental health advice, consultation, and support to acute hospital staff.
• Develop, implement, and review person-centred plans and facilitate onward referrals.
• Work collaboratively with multidisciplinary teams to ensure coordinated care.
