Job summary

Midlands Partnership NHS Foundation Trust in Crawley, United Kingdom, is seeking an experienced Practitioner for the Sussex Health & Justice Liaison & Diversion Service. You will assess adults, children and young people in Police Custody across Sussex, identifying vulnerabilities and directing care to the right services.

You will work with police, courts and healthcare colleagues to inform decisions, using clinical expertise to optimise outcomes while managing complex cases and maintaining risk

Qualifications

• Registered Mental Health Nurse, Learning Disability Nurse or General Nurse on NMC Register.
• Registered Social Worker.
• Post-registration experience with evidence of formal courses and training at diploma/degree level.
• BSc degree in nursing or equivalent level.
• Qualification in providing supervision.
• Extensive post-registration experience with mental health difficulties and complex needs.
• Experience in community settings and autonomous decision-making.
• Lived experience of criminal justice, care systems or vulnerabilities.

Responsibilities

• Deliver high-quality assessments for adults, children and young people in Police Custody settings across Sussex.
• Identify mental health, learning disability, substance misuse, physical health and social vulnerabilities early.
• Develop and implement person-centred care and support plans for complex needs.
• Make timely referrals to health, social care and community services.
• Collaborate with police, courts and criminal justice partners to support informed decisions.
• Provide accurate information about health needs to relevant agencies.
• Apply trauma-informed approaches to minimise distress.
• Manage complex cases autonomously with advanced assessment and risk management.
• Provide consultation and clinical supervision to colleagues and partner professionals.
• Promote positive outcomes by helping access the right support.
