Job summary

Qualifications

• Experience in dementia care and patient support.
• Ability to signpost and refer to services.
• Strong communication and engagement with MDT and supervision processes.

Responsibilities

• Support patients and carers in clinic after diagnosis and assist in care planning.
• Provide information and signpost to services as needed.
• Support diagnosis clinics and post-diagnosis pathways.
• Assist in cognitive assessments where required (e.g., Addenbrookes).
• Participate in MDT meetings and supervision sessions.
• Contribute to discharge planning and data collection.
