Job summary

The Memory Assessment Nurse post in Bolton is housed within the Bolton memory assessment service. You will provide specialist community and clinic-based memory assessments and guide post-diagnostic support for service users of MATS, following the Dementia clinical pathway and the MATS operating procedures.

You will work as part of a fully integrated MDT, can work autonomously and contribute to MDT meetings, with a focus on early identification and management of memory difficulties.

Qualifications

• Experienced band 6 nurse with memory problems experience.
• Registered with the NMC and eligible to practice in the UK.
• Experience in memory assessment and MDT settings is desirable.

Responsibilities

• Conduct memory assessment clinics and some home visits.
• Support diagnosis, treatment and short-term interventions for memory difficulties.
• Participate in weekly MDT meetings and contribute to service development.
• Monitor patients titrating on medications and liaise with clinicians.
