Job summary

Greater Manchester Mental Health NHS Foundation Trust is seeking a Band 6 Memory Assessment Nurse to deliver specialist memory assessments in community and clinic settings within the Bolton MAS. You will support diagnosis, treatment and post-diagnostic care while working within an integrated MDT and maintaining case-loads alongside MDT meetings.

The role requires RAMN registration, cognitive screening experience, and the ability to work autonomously within the Dementia pathway.

Qualifications

• RMN qualification with current UK registration
• Experience of cognitive screening
• Ability to prioritise and manage a small caseload
• Excellent communication and accurate record-keeping
• Ability to work across MDT and autonomously

Responsibilities

• Provide specialist memory assessments in clinics and via home visits.
• Support diagnosis, treatment and short-term interventions for memory difficulties.
• Contribute to MDT meetings and care planning.
• Manage a small caseload including monitoring treatment and liaising with GPs.
• Monitor treatment response and coordinate with medical teams on medication changes.
• Provide teaching, supervision and service development within MAS.
