Job summary

The post supports Community Mental Health Services, coordinating prescribing requests and medicines management to improve access, safety, and efficiency.

Qualifications

• Qualified Pharmacy Technician registered with GPhC.
• Post-registration experience in pharmacy services or medicines management.
• Knowledge of electronic prescribing systems and multidisciplinary teamwork.

Responsibilities

• Support day-to-day operation of Centralised Prescribing Team.
• Process and coordinate prescribed requests per Trust policies.
• Undertake medicines reconciliation and support medicines management.
• Provide prescribing and medication-related support to clinicians.
• Monitor prescribing workflows to ensure timely completion.
• Investigate and resolve prescribing queries from patients, carers and professionals.
• Support medication safety, audits and quality improvement.
• Maintain accurate electronic patient records and prescribing documentation.
• Collaborate with community pharmacies, GP practices and specialist services.
