Job summary

Great Western Hospitals NHS Foundation Trust is seeking a Medicines Optimisation Pharmacy Technician to join the clinical pharmacy team. The role focuses on medicines reconciliation, patient counselling, and medicines safety to enhance patient care within a forward-thinking department.

Qualifications

  • Knowledge of pharmaceutical technical procedures through NVQ3, BTEC or equivalent.
  • Additional training for checking dispensed prescriptions and supervising a technical area.
  • Knowledge of medicines management and policies; degree-level qualification preferred.

Responsibilities

  • Carrying out timely and accurate medicines reconciliation for patients on admission.
  • Supporting safe and effective discharge through ward-level processing and counselling patients about their medicines.
  • Participating in ward-based medicines optimisation initiatives to reduce medication errors and improve outcomes.
  • Helping implement best practice, formularies, and medicines policies across the Trust.
  • Supporting medicines supply, stock management, and minimising waste.
  • Using digital systems to improve the flow of medicines information and support clinical decision making.
  • Acting as a role model for high-quality, patient-centred medicines management.

Are you an enthusiastic and forward-thinking Pharmacy Technician looking to take the next step in your career? The Great Western Hospitals NHS Foundation Trust is seeking a motivatedMedicines Optimisation Pharmacy Technician to join our dynamic and supportive clinical pharmacy team.

In this role, you will play a key part in ensuring safe, effective, and efficient use of medicines across our hospital. Working closely with pharmacists, nursing staff, prescribers and wider clinical teams, you will support high-quality patient care by delivering accurate medicines reconciliation, improving medicines safety, and helping patients get the most from their medicines.

Job responsibilities

Carrying out timely and accurate medicines reconciliation for patients on admission.

Were looking for someone who is:

A registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).

Experienced in hospital or primary care pharmacy (though we welcome motivated candidates looking to broaden their experience).

Confident communicating with patients and multidisciplinary teams.

Organised, proactive, and able to work independently as well as part of a team.

Passionate about improving patient safety and medicines optimisation.

Person Specification
Qualifications
  • Knowledge of pharmaceutical technical procedures acquired through training, NVQ3, BTEC in pharmaceutical science or equivalent
  • Additional training for checking dispensed prescriptions, supervising a technical area within the pharmacy to degree or equivalent level
  • Knowledge of medicines management, including relevant legislation and policies acquired through NVQ3 or BTEC equivalent level qualification plus specialist training and experience to degree or equivalent level
Experience
