Job summary

The Methodist Church in Britain is recruiting a pharmacist for a 12-month post spanning community and hospital interfaces in Lewisham. The role focuses on structured medication reviews for older patients with polypharmacy, with independent prescribing and MDT collaboration.

Qualifications

• BPharm, MPharm or BSc (Pharmacy) qualification.
• Postgraduate Certificate in General Pharmacy Practice or equivalent.
• GPhC registration is essential.

Responsibilities

• Carry out comprehensive, patient-centred medication reviews for high-risk, frail, housebound patients.
• Prescribe independently within scope and advise patients, carers and professionals.
• Optimise medicines use to improve adherence and reduce harm, admissions and waste.
• Support safe transfer of care through medicines reconciliation across primary and secondary care.
• Collaborate with GP practices, community pharmacies, PCNs, the ICB and hospital services.
