Responsabilidades

• Conduct clinical medication reviews and provide recommendations to nurses and GPs.
• Review medicines post-discharge and reconcile medicines after hospital discharge.
• Triaging minor ailments and managing caseload to reduce unnecessary GP appointments.
• Deliver patient-facing clinics to address questions and improve medication adherence.
