Job summary

The Methodist Church in Britain is inviting applications for a PCN pharmacist role based in Central and West Warrington PCN. You will perform structured medication reviews for patients with complex polypharmacy and provide expert medicines advice within a multidisciplinary team.

Experience in primary care is valued, with Independent Prescriber status or CPPE pathway considered. Travel between sites may be required, and a flexible working approach is expected.

Qualifications

• Masters degree in pharmacy is required.
• Completed or in training for pre-registration year.
• Commitment to continuing professional development.

Responsibilities

• Perform structured medication reviews for complex polypharmacy patients.
• Provide expert clinical medicines advice to patients and teams.
• Support DES specifications and local practice priorities.
• Work in a patient-facing role within a multidisciplinary team.
