Northern Lincolnshire and Goole NHS Foundation Trust's Macmillan Health Care Team is seeking a part-time post(24.5 hours per week) to support leadership and coordinate care for patients approaching end of life in the community.
The post holder will supervise MHCT staff, conduct staff inductions, oversee rotas, assess competencies and manage referrals in line with care plans, under the supervision of a registered nurse.
This is an exciting opportunity for 24.5hrs per week working with the Community Macmillan Healthcare Team. The post holder will work within and as part of the Macmillan Health Care team supporting and working with both the clinical team and the administrative team to promote and facilitate effective leadership for the Macmillan healthcare team and facilitating patients who are approaching the end of their lives to achieve their preferred place of care within the community., The post holder is responsible for supervising and coordinating the MHCT Care staff and ensuring patient care is carried out according to the care plan and as delegated by the registered nurse.
The post holder will be responsible for assisting in the induction of new staff, conducting supervision of junior staff members, assessing competencies of staff within the team, completing rotas as required and managing referrals into and out of the service using specific criteria.
The post holder will carry out delegated work in accordance with the competencies detailed within the NQF at Level 5 foundation degree level. Direct patient care will be completed under the indirect supervision of a Registered Nurse according to the individual patient care plan, including discussing and reporting any changes in the patient's condition to the registered nurse in a timely manner.
Working as part of a specialty team, the post holder will offer support to ensure patients are afforded a professional and caring service within the necessary clinical timescales.
It is vital that patients are appropriately tracked through the process to ensure they receive care at appropriate times, therefore there will be an element of validation and auditing required ensuring that patients are treated in a timely and efficient manner.
The nature of this post requires the post holder to have a high level of tact and diplomacy as well as strict adherence to the rules of confidentiality.
Please note that candidates must have a level 5 qualification in Health and Social Care.
