Job summary

Gloucestershire Hospitals NHS Foundation Trust is seeking a Lung Cancer Support Worker (Band 4) to support patients through cancer treatment and beyond. The role focuses on delivering Holistic Needs Assessments, coordinating care and offering emotional and practical support across site.

You will work under the CNS, engaging with patients, carers and professionals to improve care pathways and patient safety, while promoting self-management and informed decision making.

Qualifications

• Empathy and compassionate communication with patients and families.
• Experience working in NHS or health/social care settings.
• Ability to coordinate patient workloads and navigate care pathways.
• Knowledge of cancer care and Holistic Needs Assessments is desirable.
• Use of IT systems to monitor databases and patient information.

Responsibilities

• Provide information and support to help patients navigate health and social care services.
• Daily contact with patients, relatives, carers and professionals to share information empathetically.
• Support patients with cancer across curative and palliative pathways.
• Undertake Holistic Needs Assessments and help formulate care plans.
• Signpost to services, signposting and self-management resources.
• Assist in service development and care pathway redesign with the CNS team.
