Job summary

Gloucestershire Hospitals NHS Foundation Trust seeks a dedicated role to support people navigating the cancer care journey. The post holder will coordinate care, deliver emotional and practical support, and lead Holistic Needs Assessments to identify patient concerns.

Qualifications

Previous NHS experience in patient care.
Computer literate, use of Microsoft products.
Experience of multi professional working.
Able to use own initiative and work without supervision.
English and Maths GCSE (A-C grade or 5 or above).

Responsibilities

Support people to navigate the health and social care system during and after cancer treatment.
Coordinate care by delivering emotional and practical support to patients with cancer.
Lead in Holistic Needs Assessments to identify and address patient concerns.
Signpost to information and support services and refer to appropriate teams.
Assist in redesign of care pathways to improve quality and safety.
Undertake administrative tasks such as monitoring databases and helplines.
