Responsibilities

• Conduct comprehensive assessments for individuals dealing with substance misuse issues.
• Develop and implement tailored care plans to support sustained recovery.
• Deliver evidence-based interventions and health education to patients and their families.
• Monitor and review patient progress, adjusting care plans as necessary.
• Work collaboratively with a multidisciplinary team to ensure holistic patient care.
• Provide guidance and support in harm reduction strategies.
• Maintain accurate and up-to-date patient records and documentation.
• Offer emotional support and counselling to patients handling challenging life transitions.
