Job summary

Locum Ready is seeking a NMC-registered Practice Nurse to work at a GP practice in Hyde, delivering chronic disease reviews and routine clinic activity in a structured primary care setting.

You will perform wound care, immunisations if trained, smear tests, NHS Health Checks, and maintain clear documentation in the clinical system, with flexible long-term bookings and regular sessions.

Qualifications

• NMC-registered nurse with experience in primary care.
• Immunisation training and cytology training up-to-date desirable.
• Confident in chronic disease management and treatment room work.
• Good patient communication and clinical reasoning.
• Familiar with EMIS, SystmOne or Vision.
• Enhanced DBS.

Responsibilities

• Conduct chronic disease reviews for patients with asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations (if trained).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.
