Job summary

Solent Mind is recruiting for a Peer Support Worker for the Coastal Primary Care Network Wellbeing Centre in Fareham & Gosport. The role supports patients with lived experience of mental health problems, providing one-to-one wellbeing support within GP practices and alongside the wider Primary Mental Health & Wellbeing service.

This part-time role (22.5 hours/week) requires safeguarding awareness, good IT skills, and ability to travel to PCN centres.

Qualifications

• Educated to level 2 standard or equivalent experience.
• Experience living with mental health problems and using mental health services.
• Understanding of the role of peer support within mental health.
• Experience of working with people with mental health needs, in paid or voluntary capacity.

Responsibilities

• Identify people who may benefit from peer support within the PCN.
• Manage your own caseload and support recovery.
• Help participants develop wellbeing skills and access services.
• Encourage clients to identify strengths, triggers and life goals.
• Maintain records on client management systems (CRM/SystemOne/Emis).
• Develop links and maintain relationships with local communities.
