Job summary

Action Together CIO in Tameside is recruiting four Live Well Community Navigators to join the hybrid team. You will support a busy caseload, help people access community-based services, improve health and wellbeing, and strengthen local connections.

Community Navigation Service

The Tameside Live Well Community Navigation Service helps adults aged 18 and over access the support, opportunities, and connections they need to live well. Delivered in communities across Tameside and funded by Tameside Council, the service supports people who want to make positive changes to their health, wellbeing, or circumstances by connecting them with the right help at the right time.

Our independent Community Navigators work alongside local residents, providing as much or as little support as needed. With detailed knowledge of services, activities and community resources across Tameside, the team helps individuals find personalised support that matches their needs, goals, and interests. Whether someone is looking to improve their wellbeing, access practical help, build confidence, reduce isolation, or become more connected to their community, we help them navigate the options available. By building strong links with local statutory services, community organisations and voluntary groups, Community Navigation makes it easier for residents to access support and prevent issues from escalating.

Alongside direct support for individuals, the service also invests in community development to strengthen local networks, identify gaps in provision and ensure communities have the resources needed to meet the changing needs of Tameside residents.

Focused on what matters most to the people we work with, the service helps individuals discover opportunities, build meaningful connections, and access the support they need to live well.

We are recruiting 4 Community Navigators to join the Live Well Community Navigation team.

Live Well Community Navigator

The Live Well Community Navigators will play a key role within the Live Well Community Navigation Service at a neighbourhood level. They will support a busy caseload of individuals, many of whom may have complex needs, to improve their health and wellbeing. This will involve helping people access community-based and community-led services that strengthen local connections, improve outcomes, and provide appropriate support. Community Navigators will also help individuals navigate to the right areas of the public sector system when further assistance is needed.

The ideal candidate

We're looking for people who understand that many barriers to wellbeing can be addressed within our communities and have the skills, ability and passion to work alongside people to find, and access that support. They will:

  • Be skilled collaborators, able to join things up to achieve the best outcomes, while also being proactive in developing their own knowledge and skills.
  • Be Confident working independently and managing a busy caseload.
  • Be experienced at working with people to identify their strengths and support them to achieve their goals.
  • Be experienced at building successful working relationships with partners from other agencies.
  • Have a good understanding of the health and social care system and the role of community navigation within it.
  • Have a strong working knowledge of safeguarding practices, information sharing and data protection.
