Bluebird Care Cardiff and The Vale is seeking a Live-In Care Coordinator to manage scheduling and coordination of our live-in care team across Cardiff and The Vale. You will tailor care plans, coordinate carers to clients, and ensure seamless service delivery with accurate records and compliance with CIW regulations.

Qualifikationen

Experience in scheduling live-in care and coordinating care teams.
Ability to create care plans and risk assessments.
Strong organisational and communication skills.

Aufgaben

Scheduling live-in care packages and matching carers to clients.
Creating bespoke care plans and risk assessments.
Completing reviews of care plans and risk assessments per CIW regulations.
Weekly handovers with staff and clients.
Ensuring daily coverage and updating care needs.
Processing new referrals and updating records.
Supporting the care team to succeed.
Participating in on-call rota (1 day per week, 1 weekend in 4).
