Job summary
Bluebird Care Cardiff and The Vale is seeking a Live-In Care Coordinator to manage scheduling and coordination of care assistants across Cardiff and The Vale. You will ensure seamless delivery of live-in packages, matching carers to clients and maintaining accurate records.
The role involves creating bespoke care plans, risk assessments, and weekly handovers, with on-call responsibilities. A full UK driving licence and access to your own vehicle are required.
Qualifications
- Experience in scheduling live-in care packages and coordinating teams.
- Ability to create bespoke care plans and risk assessments.
- Strong organisational skills under pressure.
Responsibilities
- Scheduling live-in care packages and matching carers to clients.
- Creating bespoke care plans and risk assessments.
- Completing reviews of care plans and risk assessments per CIW regulations.
- Weekly handovers with staff and clients.
- Ensuring full shift coverage and smooth care delivery.
- Processing new referrals and updating care needs.
- Liaising with the office team to maintain records and reports.
- Supporting the care team to succeed.
- Participating in the on-call rota (1 day/week, 1 weekend in 4).
