Job summary

Swindon Borough Council is seeking a Link Worker to support young people aged 14–25 transitioning from children’s services to adult services. You will work closely with young people, families, schools and health services to complete Care Act assessments and develop support plans before 18, then assist pathways after 18, including employment and independent living.

The role operates within the Bridge to Adulthood team, which includes social workers and care practitioners, with a duty rota element

Qualifications

• Fluent English required for customer-facing role.
• Ability to travel throughout the Borough; access to a car or use of public transport is essential.
• Hybrid working model with in-office presence required.
• Enhanced Disclosure and Barring Service (DBS) check will be carried out.

Responsibilities

• Provide one-to-one support to young people aged 14–25 in transition to adult services.
• Collaborate with the allocated social worker or assistant care manager to plan for independent living and future employment.
• Attend transitions team live events and contribute to care planning.
