Job summary

Qualifications

• Sound understanding of Care Act interventions and Strengths Based Practice, Mental Capacity, Court of Protection and Safeguarding Adults.
• Experience building relationships with partners in voluntary, community and health sectors.

Responsibilities

• Hold a caseload and implement short and long term interventions using a strengths-based approach.
• Undertake social care assessments, reviews and support planning, mental capacity assessments and safeguarding enquiries when necessary.
