Liaison & Diversion Practitioner roles within the East Lancashire region of the Lancashire & South Cumbria NHS Foundation Trust. • Experience in assessment and referral within Criminal Justice or health/social care settings.
• Ability to use validated tools for vulnerability assessment.
• Strong communication and interagency liaison skills.

Responsabilités

• Assess for vulnerabilities using validated tools and share relevant information to ensure access to appropriate services.
• Provide input on capacity for effective participation in criminal justice procedures, including cultural and gender needs, age-related factors and housing/finance concerns.
• Liaise with agencies to support onward planning and intervention, and provide brief community-based interventions where appropriate.
