Job summary

4Recruitment Services is seeking a Personal Advisor Leaving Care to join the Leaving Care Team in Bolton, Greater Manchester. You will support Children Looked After and Care Leavers aged 16-25, helping with health, education, employment and housing as they transition to adulthood.

You will manage a caseload, contribute to pathway plans, visits and reviews, and work with partner organisations, meeting statutory requirements and safeguarding expectations.

Qualifications

• Experience working with vulnerable young people aged 16-25.
• NVQ level 3 or equivalent qualification.
• Knowledge of human growth and development and issues affecting looked-after young people.
• Awareness of relevant legislation: Children Act 1989/2004, Leaving Care Act 2000.
• Ability to develop strategies when working with challenging behaviour.
• Strong interpersonal skills with young people, families and professionals.
• Enhanced DBS clearance and safe recruitment practices.

Responsibilities

• Collaborate with partner organisations to assess, plan and support care leavers and their families.
• Manage a caseload in line with policies, procedures, legislation, guidance and regulations.
• Attend and contribute to reviews and meetings for young people wherever they reside.
• Assess needs and deliver tailored interventions for a diverse group of care leavers.
• Assess financial needs and assist with planning individual budgets for accommodation and maintenance.
• Undertake visits, complete assessments and maintain records according to policy.
• Plan and record Pathway Planning meetings when required.
• Visit young people at home and other venues in line with standards and procedures.
