Job summary

Oxfordshire County Council in Abingdon is seeking a Leaving Care Personal Advisor to support care-experienced young people aged 16-25, guiding them as they transition into adulthood.

You will manage a caseload, develop Pathway Plans, assist with housing, education, employment and health, with a hybrid working pattern and a 37-hour week. This role offers a supportive team and the chance to make lasting differences in young lives.

Qualifications

• Experience supporting young people in social care, youth services, education, housing, or related settings.
• Ability to assess needs and develop effective support plans.
• Experience multi-agency working and partnership collaboration.
• Excellent organisational and case management skills.
• Passion for improving outcomes for care-experienced young people.

Responsibilities

• Manage a caseload of care-experienced young people aged 16-25.
• Develop and review Pathway Plans that reflect each young person's goals and aspirations.
• Support young people to access housing, education, training and employment opportunities.
• Advocate on behalf of young people with partner agencies and professionals.
• Coordinate services and support packages to promote independence.
• Work with young people from diverse backgrounds, including unaccompanied asylum-seeking young people.
• Maintain accurate records and contribute to positive outcomes for young people leaving care.
