Job summary

NELFT NHS Foundation Trust is seeking a dedicated Physiotherapist to provide high-quality care for adults with learning disabilities in the Community Learning Disability Team. You will work within an integrated team of health professionals and social workers, delivering assessment, treatment and education to promote independence and safety.

Role requires complex caseload management, evidence-based practice, and strong collaboration with carers.

Qualifications

• Perform competent assessments of patients using clinical reasoning and established techniques.
• Operate as an autonomous registered practitioner guided by professional and Trust standards.
• Obtain valid informed consent and work within a legal framework with patients who cannot consent.
• Develop and deliver individual therapy plans based on evidence-based practice.
• Ensure care plans are person-centred and co-produced with service users and carers.
• Produce accessible care plans and risk assessments; document thoroughly for audits.
• Refer to specialist services as needed and support integration with the wider team.

Responsibilities

• Provide high quality Physiotherapy clinical expertise and service.
• Undertake comprehensive clinical assessments of service users.
• Prioritise and manage own caseload as allocated by the lead.
• Maintain accurate patient records per Trust standards and guidelines.
• Assist senior staff with day-to-day department activities.
• Work as part of an integrated team for adults with learning disabilities.
• Travel to various sites within and outside the borough when required.
