Permanent, Full Time (37.5 hpw)

Pay £13.21 to £13.63 per hour (£25,838 to £26,645 per annum) and great benefits including Health Cash Plan

Hampshire West is a Mental Health Supported Accommodation service which also delivers a community outreach service within the same footprint. Within our accommodation Hubs in the New Forest, Winchester, and Eastleigh, we work with adults experiencing severe and enduring Mental Health challenges.

Supporting individuals to develop greater self-reliance and independence, enabling better self-management of personal mental health, physical health, and wellbeing. In the community, we support vulnerable people with complex needs through short-term interventions, linking customers to other services and professionals.

You’ll support customers with needs around hoarding, poor living conditions, or difficulty maintaining their wellbeing. You’ll make a big difference every day helping isolated customers in the community achieve their hopes and aspirations. Amazing, we know!

Typical day as a Lead Support Worker (known as Coordinator)

  • Working with customers with complex needs in their homes, encouraging engagement and building trust with us and other agencies.
  • Facilitating access to work, education, or training.
  • Creating personal support plans and risk assessments with customers, organizing regular reviews, and coordinating support with other professionals.
  • Supporting customers to manage substance misuse issues, thoughts/feelings about suicide/self-harm.
  • Helping customers with daily activities such as making meals, shopping, budgeting, maintaining tenancy, and moving to independent accommodation.

You bring

  • A passion to support customers to live their best lives, with collaborative working and attention to detail.
  • Experience in creating person-centred support plans and supporting colleagues to implement them.
  • Experience in coordinating and assessing customer referrals, making impactful first contact.
  • The ability to work independently, manage your diary, stay calm under pressure, and be resilient.
  • A vehicle insured for business purposes (we pay mileage).

You’ll join Emma and the Hampshire West team, a friendly group committed to supporting each other during monthly meetings, always with snacks and a brew.

We’re passionate about our work, supporting customers who have fallen through gaps to get back on their feet and connect with other services.

Job details

  • Flexible work patterns to meet customer needs, including occasional adjustments to start or finish times.
  • Accommodation service operates Monday to Friday 9-7 and weekends/bank holidays 10-6.
  • Outreach service primarily Monday to Friday, with flexible hours as needed.
  • Regular team meetings and training, despite occasional lone working.
  • Proficiency with technology for support plans, online learning, and collaboration.
  • Enhanced DBS check required (paid by us).
