Description

Lead Support Worker

Primarily in the East (Seaham, Horden, Peterlee) but also some work covering the whole of County Durham.

Pay £13.21 per hour (£25,838 per annum) [pro rata] and great benefits including Health Cash Plan.

As our Lead Support Worker, you’ll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
  • We are a mental health service providing group setting support and also a 1:1 befriending service. Your role will include planning sessions for the groups and actively promoting the groups, allotments and the befriending service.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Job details

  • Tuesday - Friday 9am-5pm or Monday - Friday hours to be discussed.
  • We are flexible with your schedule, but we need to meet the needs of our customers. This may mean adapting your start or finish times occasionally.
  • Able to use technology for creating and updating support plans, completing online learning, and collaborating with colleagues.
  • You’ll need an Enhanced DBS check, which we will pay for.
