Job summary

Turning Point is launching a brand-new residential rehabilitation and Discharge to Assess service, The Stewarts, to support people with complex mental health needs towards independence and successful move-on outcomes.

The Transition Lead role blends recovery work with leadership to oversee admissions, move-on planning and discharge pathways from start to finish, ensuring goals and accommodation plans are embedded early and throughout care.

Qualifications

• Experience in mental health, social care, supported housing or community services.
• Strong recovery-focused and person-centred approaches.
• Experience supporting transitions, move-on or discharge planning.

Responsibilities

• Lead admissions, transition and move-on planning across the service.
• Begin move-on planning at admission and keep it central.
• Develop personalised transition plans with individuals.
• Coordinate pathways to supported accommodation, independent living and community services.
• Build relationships with housing providers, local authorities and partners.
• Identify barriers delaying moves and impact on outcomes.
• Monitor bed flow, vacancies and transition outcomes.
• Support people through periods of change, building confidence.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Record data, contribute to audits and continuous improvement.
