The Methodist Church in Britain is seeking a Lead Practitioner for the Horsham Early Intervention in Psychosis service. The role focuses on care coordination for a caseload of 14–65-year-olds with first episode psychosis or at-risk mental state, from referral to discharge for up to three years.

Some remote working is available within a multidisciplinary team. Registration as a nurse, social worker or occupational therapist is required, with mentorship training and ongoing CPD expected to support

Formación

• Completion of a mentorship course or ENB equivalent.
• Evidence of continuous professional development.

Responsabilidades

• Provide care coordination for a small caseload of clients aged 14 to 65 with suspected or confirmed first episode psychosis or an at‑risk mental state, from referral to discharge for up to three years.
• Develop therapeutic relationships and person‑centred care and treatment plans to reduce distress, maintain independence, prevent relapse and promote recovery and social inclusion.
• Work with friends and family as a key part of how care is delivered.
• Work in partnership with clients with complex health and social care needs, using NICE recommended interventions and an assertive outreach approach.
• Work closely with carers, families and partner agencies.
• Promote the safety and wellbeing of service users and their families and carers.
• Triage and assess referrals to the service.
• Assess, plan and implement care.
• Provide specialist advice and carry out specialist assessments.
