Courtyard Surgery invites an experienced Lead Practice Nurse and Independent Prescriber to join our rural GP practice on a part-time basis. The role focuses on long-term condition clinics, immunisations, cervical screening and autonomous prescribing while maintaining high standards of clinical governance.

The successful candidate will lead and develop the nursing team, run nurse-led clinics, and support the GP partners in delivering comprehensive care to around 3,300 patients.

Responsabilités

• Lead and develop the nursing service, providing clinical leadership and supervising the wider nursing and clinical support team
• Run nurse-led clinics for diabetes, asthma, COPD, hypertension and cardiovascular disease, including annual reviews, care planning and treatment optimisation
• Prescribe independently within scope of practice to reduce GP workload
• Deliver childhood, adult and travel immunisations and lead seasonal vaccination programmes
• Provide cervical screening and cytology services
• Complete medication reviews, support repeat prescribing and manage clinical medication queries
• Provide health promotion and preventative care
• Lead quality improvement initiatives and clinical governance
• Support achievement of QOF and other contractual requirements
• Maintain oversight of infection prevention and control
• Contribute to CQC compliance, working with the GP Partner and Practice Manager
• Champion evidence-based practice and help shape the future development of clinical services
