Job summary

Kent Community Health NHS Foundation Trust in Ashford is seeking an experienced Band 6 Lead Pharmacy Technician to support frailty urgent care services, enabling hospital-level care at home. You will oversee medicines management, reconciliation and stock control while collaborating with multidisciplinary teams to prevent admissions and support safe, effective patient care.

The role requires post-qualification medicines optimisation experience, GPhC registration and the ability to mentor

Qualifications

• Registered Pharmacy Technician with the General Pharmaceutical Council.
• BTEC in pharmaceutical science or NVQ Level 3 in pharmacy services or equivalent.
• Accredited qualification in Medicines Management or equivalent.
• Post-qualification medicines optimisation experience in hospital or clinical settings.
• Experience training colleagues and mentoring others.
• Experience undertaking audits and contributing to quality improvement.

Responsibilities

• Medicines reconciliation, review and counselling, remotely and face-to-face.
• Attendance at multidisciplinary board rounds.
• Assess medication adherence and implement interventions.
• Identify and refer priority patients for pharmacist review.
• Independently resolve medicines-related queries, escalating as needed.
• Liaison withStakeholders during transfer of care to support medication management.
• Oversight for medication stock management and supply.
