We have an exciting opportunity at NMUH within the Diabetes and Endocrinology team. We are looking to recruit a Lead Nurse for our Diabetes and Endocrinology CNS team. The ideal candidate will be experienced, motivated, passionate about improving patient safety, experience, and outcomes.

This is a clinical leadership role offering scope for innovative work with various stakeholders to enhance patient outcomes and experience. The role includes supporting the development of the CNS team, including prescribing capabilities.

Main duties of the job

The Lead Nurse will operate at an advanced level, making highly complex decisions with a high degree of autonomy. They will be a competent, visible autonomous practitioner across the four pillars of CNS practice: Clinical Practice, Leadership, Management, and Education/Research.

The post holder will have clinical autonomy to assess, evaluate, and work with patients with complex Diabetes and Endocrine conditions, formulating care plans and collaborating with community and NHS Trusts.

They will develop strategies with the medical team to improve patient experience and care quality, reduce admissions, and shorten hospital stays. The role involves supporting audits, assisting ward staff, and collaborating with community services to ensure seamless care and improved patient experience.

Detailed job description and main responsibilities

  • Manage patients eligible for insulin pumps, liaising with the medical team and multi-professional staff.
  • Maintain effective communication with patients, carers, and professionals to ensure seamless service delivery.
  • Assess new patient referrals, working with consultants to ensure investigations and management plans are timely and appropriate.
  • Perform comprehensive patient assessments, including history, physical exams, and diagnostic tests.
  • Document assessments fully and communicate findings effectively to the team.
  • Review test results, identify abnormalities, and escalate as needed.
  • Coordinate patient care with consultants and administrative staff, including prescriptions and updates to care plans.
  • Provide specialist support and practice to patients and carers, ensuring informed consent and understanding.
  • Set, implement, and monitor standards of care for Diabetes and endocrine diseases.
  • Lead nurse-led clinics and review inpatient and ED patients for further treatment and community support.
  • Develop SOPs for nurse-led discharge processes.
  • Manage medication systems for safety and efficiency.
  • Work to optimize waiting lists, reduce ED wait times, and inpatient length of stay.
  • Lead self-management education for patients preparing for discharge or outpatient care.
  • Utilize non-medical prescribing within scope of practice.
  • Support projects related to insulin pumps and other patient benefits.
  • Coordinate nurse-led services for safe patient pathways, collaborating with clinical leads and service teams.
  • Collect and analyze data on patient pathways and outcomes, tracking key metrics and participating in research and quality improvement initiatives.
  • Identify risks, escalate issues appropriately, and maintain a safe environment.
  • Handle complaints locally and escalate unresolved issues.
  • Ensure adherence to policies and procedures, monitor standards, and report incidents.
  • Provide clinical leadership, develop policies, and promote evidence-based practice.
  • Participate in service development, audits, and multidisciplinary meetings.
  • Advocate for patients, ensuring their needs and preferences are respected.
  • Maintain accurate documentation and comply with governance standards.
  • Contribute to the development of protocols and pathways, both locally and nationally.
  • Lead initiatives to advance nursing and clinical practice, engaging in research and service improvement.
  • Build relationships with medical and surgical teams to foster collaboration.
  • Manage resources, including medications and supplies.
  • Lead patient education on diagnosis, treatment options, and self-management.
  • Act as a patient advocate, applying holistic and socio-economic considerations.
  • Stay informed of legislation and NICE guidelines, implementing as appropriate.
